New Patient Registration

In order to register you need to live within our catchment area – please call us or use the check your postcode map on this page if you wish to confirm that you live within this boundary. If you live outside this area, your registration may be rejected. This does not apply to UAL students.

We aim to process registrations forms within 5 working days of reciept.

We will contact you to acknowledge receipt of your registration request. To complete your registration we ask all patients to provide proof of address. If you are unable to provide this documentation at the time of registration, we will require you to provide this within one month of your registration.

If you prefer not to register online please complete the following steps:

  1. Please check our map (above) to ensure you live in the catchment area
  2. Download, print and complete the NHS GP patient registration form, also available from reception.
  3. Return your completed registration forms to reception or post to us
  4. If you don’t have an access to a printer and would like us to post a registration form to you please give us a call on 0207 987 4231

Named GP

Practices are required under the GMS contract, to allocate a named, accountable GP to all patients, including children who will have overall responsibility for the care and support that our surgery provides to you. All new patients registering will be informed about their named GP at registration. Please ask the reception staff, who your named, accountable GP is. Should you express a wish for this to be changed to another GP we will do our best to accommodate your wishes. Having an allocated GP does not prevent you seeing any other doctor in the practice.
